Keeping records is a valuable life skill.  The type of form used is not as important as the skill learned.  The Montana State 4-H program created forms in 2012 for 4-H members to use.  Those forms are below.  They can be printed and handwritten, or downloaded as an electronic file and updated using a computer.

Each member should have:

  • One My 4-H Year Form
  • A Project Journal for each project they are taking that is a non-animal project.
  • An Animal Project Journal for each animal project they are taking.

If you have questions regarding record book completion, please contact your club leader or our office at teton@montana.edu
